About the Role

We are seeking a highly motivated and caring Mental Health Practitioner to join our Community Older Adults Mental Health Team. As a key member of our team, you will be responsible for assessing and implementing intensive care and treatment for older adults with functional mental health problems and dementia.

Key Responsibilities
  • Manage a caseload of older adults with mental health needs, working closely with carers and other professionals to assess and implement care plans.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
  • Provide specialist consultation and advice on issues related to the care of older adults with mental health problems to other professionals within the wider care community.
  • Develop and maintain knowledge of therapeutic interventions, including crisis intervention and problem-solving.
