About the Role

We are seeking a motivated and experienced Community Mental Health Practitioner to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Older Adult Admission Avoidance & Home Treatment Team, you will play a vital role in supporting older adults with functional mental health issues to meet increased needs and rising risk to support admission avoidance and provide home treatment with a Home First approach.

Key Responsibilities

  • Work with a new and growing team, existing community services, and established Dementia Admission avoidance services across Mid and South Essex to provide comprehensive support to older adults.
  • Develop and implement effective care plans to meet the needs of older adults with functional mental health issues.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Provide education and training to staff and patients on mental health and well-being.
  • Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.

Person Specification

We are looking for a candidate with:

  • Registered Mental Health Nurse or Occupational Therapist qualification.
  • Additional professional qualifications in Older Adult Mental Health/Dementia or experience.
  • Evidence of personal and professional development in contemporary mental health practice and dementia care.
  • Good understanding of relevant legislation and guidelines, including the Mental Capacity Act and Care Act.
  • Knowledge of national policy and its implications for good mental health.
  • Ability to build relationships and negotiate satisfactory outcomes.
  • Ability to use own judgement confidently and decisively.
