About the Role

We are looking for a dedicated and compassionate individual who is passionate about delivering exceptional patient care. The successful candidate will be responsible for assessing patient needs, developing personalized care plans, and coordinating services across multiple providers.

Key Responsibilities
  • Assessment & Planning:
    • Conduct thorough initial and ongoing assessments to determine patient care needs.
    • Develop holistic care plans that address medical, physical, and emotional needs.
    • Coordinate services with various healthcare providers to ensure comprehensive care.
  • Care Coordination:
    • Liaise with healthcare professionals to ensure patients receive timely and effective care.
    • Provide regular communication with patients and families regarding care updates, changes, and instructions.
  • Monitoring & Evaluation:
    • Regularly monitor patient progress and make adjustments to care plans as necessary.
    • Evaluate patient outcomes and ensure care aligns with clinical guidelines and best practices.
  • Support & Advocacy:
    • Act as the primary point of contact for patients and families, offering support and education on the care process.
    • Help patients understand treatment options, insurance coverage, and rights.
  • Documentation & Reporting:
    • Maintain accurate records of patient interactions, care plans, and medical history.
    • Ensure compliance with healthcare regulations, patient privacy laws, and organizational policies.
Requirements

What We Offer

Austin Fraser offers a competitive salary of $65,000 - $85,000 per annum, depending on experience, as well as a range of benefits including health insurance, retirement plan, and opportunities for professional development.
