Job Title:

A Temporary Support Worker is required to provide 5-hour per week support, Monday to Friday from 9AM to 5PM.

This temporary role is expected to last approximately 1-2 months with the possibility of extension based on performance.

Key Responsibilities:
  • To co-produce strengths-based and outcome-focused support plans with clients, including assessment and planning of their needs.
  • To assess client risk and develop strategies for managing it in collaboration with the client, housing provider, and other relevant services.
  • To identify and assess physical and mental health needs, facilitating access to healthcare services and promoting overall wellbeing.
  • To assist clients in identifying and accessing activities that enhance their quality of life, such as work opportunities, education, and social inclusion programs.
  • To participate in organizing and delivering activities, drop-in sessions, or groups tailored to client needs or as part of a project-wide initiative.
  • To report and act upon safeguarding concerns for children and adults at risk of harm or neglect.
Minimum Requirements:
  1. Relevant experience or understanding of approaches for working with individuals with specific needs, including mental health, physical wellbeing, drug and alcohol use, safeguarding, and social inclusion.
  2. An understanding of client needs assessment, onboarding, and move-on planning.
  3. Knowledge of common health issues affecting people experiencing homelessness and barriers to healthcare access.
  4. Knowledge of housing pathways and move-on routes for individuals transitioning from supported accommodation to independence.
  5. The ability to use personalized assessment and support planning tools, including motivational interviewing and strengths-based interventions.

Estimated Salary: £18.55 - £20.12 per hour
