Mental Health Nurse Specialist

Essex Partnership University NHS Foundation Trust is seeking a skilled Mental Health Nurse to join our team. As a key member of our community care team, you will be responsible for providing high-quality, evidence-based nursing care to individuals in need.
About the RoleThis is a fantastic opportunity to work in a supportive and well-resourced team, with a focus on delivering person-centered care to older adults. Your caseload will primarily be based in Brentwood, where you will work closely with patients, carers, clinicians, and other practitioners to deliver services.
Main Responsibilities- You will assess, plan, implement, and evaluate packages of care based on individual service user needs, under the guidance of the Team Manager/Clinical Lead.
- You will provide advice and decisions to other service members and partner agencies in relation to crisis situations, conditions, and treatment plans, based on medical history and knowledge of certain conditions.
- You will conduct/contribute to the assessment process, as well as the observation and ongoing assessment, providing accurate and timely feedback.
To be successful in this role, you will need:
- A strong understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983 and safeguarding.
- The ability to work collaboratively with clients, service colleagues, and other agencies to prevent inappropriate A&E attendances and Acute Hospital admissions.
- Excellent communication skills and the ability to work closely with families and carers as a routine part of care.
