Job Overview

We are seeking an experienced and skilled Senior Therapist to join our team as a Band 7 Clinical Team Lead for our Discharge Services. This is an exciting opportunity to work with a multidisciplinary team, providing autonomous clinical and professional decision-making to support the planning of complex discharges.

The successful candidate will have experience in managing complex cases, staff and resource management, and a strong understanding of common medical and surgical conditions. They will also have a thorough knowledge of Discharge To Assess Pathways and The Hospital Discharge Service: Policy and Operating Model.

This role offers the ideal chance to shape the service and be part of shaping the future of healthcare. As a key member of the team, you will work closely with other health and social care professionals to deliver seamless services and ensure that discharge planning is delivered to the highest possible standard.

About the Role
  • Lead a multidisciplinary team in the planning and delivery of complex discharges
  • Provide autonomous clinical and professional decision-making to support discharge planning
  • Develop and implement effective strategies to improve patient outcomes and reduce readmission rates
  • Collaborate with other health and social care professionals to deliver seamless services
Key Requirements
  • Qualified to degree level or equivalent evidence of post-registration qualifications/professional development in a related health field
  • Therapy-based Degree/Diploma
  • Registered with Health Professional Council
  • Experience of working with occupational therapy/physiotherapy client groups plus further theoretical study and experience within a community environment to master's level equivalent
Salary and Benefits

The salary for this position is £54,992 per annum, plus benefits including a contributory pension scheme, generous annual leave entitlement, and opportunities for continuing professional development.
