About the Role

We are seeking a Health Care Support Worker to join our Enhanced Recovery and Support at Home team in Gosport and Havant. As a key member of our multidisciplinary team, you will provide person-centred care to patients in their own homes, working closely with OTs, physiotherapists, and nurses to deliver community-based support.

Key Responsibilities
  • Deliver person-centred care according to patient care plans under the supervision of qualified staff
  • Support patients with daily living tasks, mobility, and rehabilitation
  • Work collaboratively with the multidisciplinary team to achieve patient goals
  • Communicate effectively with patients, families, and healthcare professionals
Requirements
  • Evidence of a good standard of general education and legible handwriting
  • Linguistic competence in English
  • Access to a motor vehicle and a clean driving license
