About the Role
Southern Health NHS Foundation Trust is seeking a skilled and compassionate Healthcare Support Worker to join the GOSPORT Community Care Team. As a key member of the team, you will play a vital role in delivering high-quality patient care under the supervision of the Community Care Team.
Main Responsibilities
- Participate in the delivery of patient care, following written care plans and care pathways devised by the Registered Nurse.
- Work under supervision to provide care to housebound patients with complex care needs, enabling them to receive the care they need in their own home.
- Follow prescribed care plans and treatment protocols, and report any variances to the Registered Nurse.
- Delegated responsibility for Patient Support Teams admin.
