About the Role

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at John Hampden Surgery. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our practice.

Key Responsibilities
  • Diagnose and Treat Patients: Conduct thorough assessments, diagnose, and treat patients with minor illnesses and acute conditions, making referrals to other healthcare professionals as necessary.
  • Manage Chronic Conditions: Provide ongoing care and management for patients with chronic conditions, including developing and implementing personalized care plans.
  • Provide Health Education and Advice: Offer health education and advice to patients on various topics, including healthy living, disease prevention, and management of chronic conditions.
  • Conduct Health Screenings: Perform routine health screenings, including blood pressure checks, urinalysis, and other tests as required.
  • Administer Medications and Treatments: Administer medications and treatments as prescribed by GPs, including vaccinations, immunizations, and other medical procedures.
  • Maintain Accurate Records: Ensure accurate and up-to-date records of patient consultations, treatments, and outcomes.
  • Collaborate with Healthcare Professionals: Work closely with GPs, other healthcare professionals, and support staff to ensure seamless patient care and effective communication.
Requirements
  • Qualifications: Hold a Qualified Nursing Qualification and be registered with the NMC.
  • Experience: Have experience working in a primary care setting, preferably in a GP practice or community healthcare setting.
  • Skills: Possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Knowledge: Have a strong understanding of primary care principles, including minor illness and acute care, chronic condition management, and health education.
