Job Title: Advanced Healthcare Practitioner

A high-profile, accessible, and responsive leader with managerial and clinical responsibility for delivering advanced clinical practice across cardiology and the emergency and acute footprint.

Clinical Responsibilities
  • Coordinate care for cardiology patients throughout their pathway with the multidisciplinary team and clinical services to ensure effective management of this group of patients.
  • Lead and deliver clinical care across various settings according to national and local guidelines, ensuring delivery of evidence-based care and working towards the Trust vision of providing a World Class Service.

Key responsibilities include:

  • Delivering care according to local, national guidelines, and quality standards to ensure delivery of evidence-based, safe, quality care.
  • Providing leadership to appropriate staff in the service to deliver high-quality and effective care, ensuring the needs of patients are a priority.
  • Monitoring the performance of team members, identifying, and implementing actions to achieve/maintain standards of care in line with organisational policy.
  • Initiating and participating in the maintenance of quality governance systems and processes across the organisation and its activities.
  • Continuously assessing and monitoring risk in their own and others' practice and challenging about wider risk factors.
  • Using a structured framework (e.g. root-cause analysis) to manage, review, and identify learning from patient complaints, clinical incidents, and near-miss events.
  • Advising and contributing to investigations following complaints and critical incidents.
  • Managing specifically identified services or projects as agreed.
  • Contributing to the development of local guidelines, protocols, clinical pathways, and standards.
