Job Description

We are seeking a skilled Healthcare Support Specialist to join our team in providing high-quality care and support to patients in their own homes.

The successful candidate will work closely with healthcare professionals to assess patients' needs, provide ongoing care and support, and promote patient health and wellbeing.

Key responsibilities include:

  • Conducting first visit assessments of patients in their own home
  • Providing ongoing care and support to individuals
  • Assisting patients with basic needs, including prompting and administering medication as required
  • Performing physical observations using necessary equipment
  • Maintaining accurate records and completing relevant documentation

Salary: £25,000 - £30,000 per annum
