Palliative Care Clinical Lead

**Job Description**
As a Senior Staff Nurse, you will play a crucial role in our team, leading the planning, implementation, and evaluation of individual care plans for patients and their families. You will work proactively to support patients who wish to be cared for at home, collaborating closely with the ward and duty nurse to facilitate effective admission and discharge planning.
To succeed in this position, you will require excellent communication skills, a strong ability to work under pressure, and a passion for delivering high-quality nursing care. Key Responsibilities:**
- Ensure the delivery of high-quality specialist palliative nursing care.
- Supervise and participate in the delivery of evidence-based nursing care.
- Lead in the planning, implementation, and evaluation of individual care plans.
- Work proactively to support patients who wish to be cared for at home.
Essential Requirements:**
- At least three years post-RN qualification experience within a relevant specialist field.
- Working towards post-registration qualification in Palliative Care.
- Mentorship qualification or willingness to work towards.
- Evidence of commitment to ongoing professional development.
- Full driving license and access to a car.
