Activity & Wellbeing Coordinator

Job Summary:
We are seeking a highly motivated and enthusiastic Activity & Wellbeing Assistant to join our Magic Moments Team at Four Seasons Health Care. As a key member of our team, you will play a vital role in delivering high-quality activities and care to our residents, promoting their independence, wellbeing, and social engagement.
Key Responsibilities:
- Assist in the assessment of residents' needs through meetings with residents, team members, and family members.
- Support the delivery of stimulation and exercise programs tailored to individual residents' needs, including music, exercise, and arts & crafts activities.
- Build relationships with residents and families to gather life story information and develop personalized care plans and activities.
- Collaborate with care staff and relatives to create engaging activities that reflect residents' interests and hobbies.
- Develop community links with local groups and arrange trips and excursions to promote residents' involvement in activities outside the home.
- Support the roll-out of Magic Moments initiatives and encourage others to participate.
- Assist in tracking activity engagement through digital and written methods.
Requirements:
- Confident in delivering group and one-to-one activities.
- A team player with excellent communication skills.
- A genuine interest in promoting independence and lifestyle opportunities for the elderly.
- Energetic and hard-working with basic IT skills.
- Flexible and adaptable to change.
- Passionate about delivering superior services and making a positive impact.
