Mental Health Specialist

At Kent and Medway NHS and Social Care Partnership Trust, we are seeking a skilled Community Psychiatric Nurse to join our Crisis Resolution Home Treatment Team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to individuals and their families, promoting recovery-oriented principles and practice.
Key Responsibilities:- Work collaboratively with our multidisciplinary team to provide client-centered care, utilizing evidence-based knowledge and general mental health principles.
- Establish and maintain therapeutic relationships with service users and their relatives, ensuring effective communication and support.
- Assess capacity and obtain informed consent from service users, working within a legal framework when necessary.
- Develop and implement individual risk assessment and management plans, in conjunction with colleagues and in line with service and Trust policy.
- Provide mental health assessments and interventions, clearly managing risks and promoting choice and independence.
- Lead joint reviews for patients with complex needs, planning transfer of care and ensuring seamless transitions.
