Advanced Practitioner

Job Summary:

We are seeking an experienced and skilled Advanced Practitioner to join our team at Ash Grove Medical Centre. The successful candidate will be responsible for providing high-quality patient care, working collaboratively with other healthcare professionals, and contributing to the development of our practice.

Key Responsibilities:

• Conduct comprehensive clinical assessments and examinations to diagnose and manage acute and chronic conditions in patients.
• Develop and implement treatment plans in collaboration with patients, families, and other healthcare professionals.
• Provide expert advice, education, and support to patients and families to promote healthy lifestyles and self-care.
• Collaborate with GPs and other healthcare professionals to ensure integrated care and improve patient outcomes.
• Contribute to the development of care pathways and quality improvement initiatives to enhance patient experience and care delivery.

Requirements:

• A strong background in clinical practice, with a focus on advanced practice and leadership.
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
