About the Role

We are seeking an experienced and skilled professional to join our Integrated Discharge Team as a Team Lead. The successful candidate will be responsible for coordinating and managing the discharge process for patients with complex mental health needs.

Key Responsibilities
  • Screen patients for complex needs and facilitate early discharge
  • Complete comprehensive assessments for patients requiring rehabilitation and funded placements
  • Develop and implement discharge plans in collaboration with the multi-disciplinary team
  • Provide leadership and guidance to the team to ensure effective discharge processes
