Integrated Discharge Team Lead

The Norfolk & Suffolk Foundation NHS Trust is seeking an experienced professional to join our Integrated Discharge Team as a Team Lead. This role will play a crucial part in improving discharge arrangements for patients with complex needs, working collaboratively with a multi-organisational team to develop processes and promote alternatives to restricted environments.
Main Duties- Screen all admitted patients to determine complex needs, working with ward staff and social care colleagues to facilitate early discharge.
- Complete comprehensive assessments for patients with complex mental health needs, using standardised psychometric measures and interpreting outcomes to inform joint decisions about future care management.
- Develop and implement processes to support safe and sustainable discharge, working with the wider multi-disciplinary team to balance risk and responsibility.
