About the Role

We are seeking an experienced and dedicated professional to join our team as an Independent Living Coordinator. In this role, you will work closely with older people who have had recent hospital admissions to support them in maintaining their independence at home.

The ideal candidate will have excellent communication and interpersonal skills, be highly organized, and be able to prioritize tasks effectively. They will also be passionate about maximizing older people's income and enjoy working in a constantly evolving environment.

As an Independent Living Coordinator, you will have the opportunity to make a real difference in the lives of older people. You will facilitate access to services, resources, and facilities, manage a caseload of customers, and respond to high levels of demand for our services.
