Senior Community Care Nurse

About the Role:

We are seeking a skilled and compassionate Senior Community Care Nurse to join our team in Bristol. This is a challenging yet rewarding role that requires strong clinical skills, excellent communication, and a passion for delivering patient-centred care in a community setting.

Key Responsibilities:
  • Clinical Care
    • Deliver high-quality nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
    • Conduct holistic assessments of patients' physical, emotional, and social needs to develop individualised care plans.
    • Monitor and evaluate patients' progress, adapting care plans as necessary to achieve the best outcomes.
    • Provide health education and advice to patients and carers to support self-management and promote independence.
  • Community-Based Care
    • Work autonomously in patients' homes and community settings, ensuring safe and effective care delivery.
    • Manage a daily caseload, prioritising tasks and responding to changes in patients' conditions.
    • Use digital tools and systems to plan, record, and report on care delivery in line with organisational policies.
  • Team Collaboration
    • Work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care.
    • Participate in multidisciplinary team meetings and case conferences to support care planning and review.
    • Act as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making.
    Requirements:
    • Registered Nurse (Adult) with current NMC registration.
    • Clinical experience in a relevant setting, such as community, primary care, or acute services.
    • Ability to work independently and manage a caseload in a community environment.
    • Strong clinical assessment and decision-making skills.
    • Excellent communication, interpersonal, and organisational skills.
    • A full UK driving licence and access to a vehicle for work purposes (if applicable).
    • A commitment to patient-centred care and promoting independence.
