The successful candidate will need to:

  • Be part of a service that is dedicated to working closely with family members as well as the client.

  • Focus on patients' recovery and be caring, imaginative, and take the initiative with ideas for improving the service.
  • Have an interest in developing CBT-focused skills and motivated about service development.

This post involves weekend and bank holiday working on a rota basis as per service user need. A car driver and use of car within work hours are essential.

We are currently working closely with other NHS Trusts to combine all our collective community, mental health, and learning disability services, with the plan to create a new larger, more integrated and accessible organisation.

As part of this transformation, we are looking for a skilled and compassionate Care Co-ordinator to join our team and contribute to shaping the future of healthcare in Hampshire.

This is an excellent opportunity to develop your skills and knowledge in a specialist team, working closely with clients and their families to achieve positive outcomes.

Person Specification

The ideal candidate will have:

  • A registered mental health nursing background or equivalent experience.
  • Some knowledge of psychosis and ability to work flexibly and independently.
  • A full driving license and willingness to use a car for work purposes.
