Social Worker Hospital Pathways Specialist

We are seeking a highly skilled and compassionate Social Worker to join our Hospital Pathways Team in Adult Social Care. As a key member of our team, you will play a vital role in empowering adults to live independently and make informed decisions about their care.
As a Social Worker in this role, you will conduct assessments, manage complex cases, and provide high standards of care. You will work closely with NHS colleagues to facilitate safe, timely discharge and enable individuals to return to their own homes, utilizing reablement and other services to regain independence.
We are looking for an individual with outstanding communication skills, a compassionate manner, and the ability to effectively manage high levels of emotional distress. You will join a welcoming, supportive, and energetic team dedicated to providing first-class supervision and ongoing professional development opportunities.

About YouWe are looking for a qualified Social Worker with current registration with Social Work England and eligibility to work in the UK. You will have 2 years post-qualifying experience working in adult services, assessing and supporting individuals with diverse needs, and successful completion of your ASYE. You will also have sound knowledge of relevant legislation, including the Care Act and Mental Capacity Act, as well as social work theories.
