Advanced Practice Nurse

We are seeking an experienced and skilled Advanced Practice Nurse to join our team at Northamptonshire Healthcare Foundation Trust. As an Advanced Practice Nurse, you will be responsible for providing high-quality care and support to individuals with mental health needs in the community.
The role involves working closely with service users, their families, and other healthcare professionals to develop and implement personalized care plans. You will also contribute to the delivery of the Community Mental Health Service by using evidence-based practice and adhering to national standards.
Key responsibilities include:
- Providing treatment and support to service users who require depot medication and Clozaril treatment
- Ensuring care plans are co-produced with service users and carers, and kept up to date and relevant
- Contributing to the delivery and development of the Community Mental Health Service
- Liaising and sharing information with others involved in service user care, in line with the Consent to Share Information Policy
We offer a competitive salary of £41,000 - £53,000 per annum, depending on experience, and a range of benefits, including a pension scheme and opportunities for professional development.
