About the Role

We are seeking a highly motivated and compassionate Support Worker to join our team at Navigo Health and Social Care CIC. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities
  • Provide practical support with daily living activities, such as personal care and household tasks.
  • Work collaboratively with service users, their families, and other healthcare professionals to develop and implement personalized care plans.
  • Assist service users in accessing community resources, including benefits and welfare rights.
  • Monitor service users' progress, level of functioning, and mental state, and alert relevant staff to any concerns.
  • Develop and maintain effective relationships with service users, their families, and other stakeholders.
Requirements
  • Experience of working with complex mental health problems and substance use.
  • A commitment to ongoing personal and professional development.
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly and respond to changing needs.
  • Basic knowledge of the Mental Health Act, Mental Capacity Act, and Community Treatment Orders.
