We are seeking a skilled professional to fill the role of Public Health Care Practitioner within our Northampton-based team.

Main Responsibilities:
  • To identify and assess the healthcare needs of infants, children, and families, providing targeted interventions where necessary.
  • To take lead responsibility for safeguarding concerns, collaborating with colleagues to ensure the child's voice is represented in multi-agency planning.
  • Take ownership of delivering high-quality care, maintaining accurate records through effective use of technology.
  • To contribute to the enhancement of service delivery, working collaboratively with colleagues to achieve quality outcomes for families.
About the Role:

This role requires a highly skilled individual to work as part of an integrated team, contributing to the delivery of quality outcomes for families. Key responsibilities include identifying and addressing health needs within communities.
