About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Oxleas NHS Foundation Trust. As a Clinical Pharmacist in Prisons, you will play a vital role in providing high-quality patient care and contributing to the delivery of our prison healthcare services.

Key Responsibilities
  • Provide clinical pharmacy services to patients in a prison setting, including medication reviews, clinical medication reviews, and face-to-face clinics.
  • Work closely with healthcare teams to identify patients at risk from high-risk medicines and implement strategies to minimize such risks.
  • Develop and manage new services that are built around new medicines or NICE guidelines, including de-prescribing and running long-term conditions clinics.
  • Provide leadership to the healthcare team to ensure practice is compliant with CQC standards where medicines are involved.
  • Work with the Specialist Prison Services Pharmacist to develop and enforce the formulary.
  • Support the audit process and disseminate findings to improve prescribing practice.
Requirements
  • Masters Degree of Pharmacy or equivalent.
  • Registered with the General Pharmaceutical Council of GB (GPhC).
  • Diploma in General Pharmacy Practice/Clinical Pharmacy/Mental Health or equivalent (or working towards).
  • Registered Independent Pharmacist Prescriber.
  • Previous prison/hospital/community pharmacy experience in the UK.
  • At least 4 years experience of providing clinical pharmacy services.
  • At least 2 years experience of providing clinical services within prisons or GP practices.
