About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our Hospital at Home Team as a Band 6 Sister/Charge Nurse. This is an exciting opportunity to work in a unique and challenging environment, providing high-quality patient care in the community and hospital setting.

Key Responsibilities
  • Assess patient care needs and develop individualized management plans using a problem-solving approach and evidence-based care.
  • Provide skilled nursing services for patients who are suitable for discharge and to continue their treatment at home.
  • Develop effective working relationships with Primary and Secondary Care colleagues, GPs, and other Primary Health Care Teams to facilitate integrated care and enable early supported discharge.
  • Support and participate in the development of integrated pathways and care packages that enable patients to be safely cared for outside of the acute hospital setting.
  • Provide clinical leadership and clinical management of the service 24 hours a day, 7 days a week, and continuously monitor and maintain clinical standards and quality care.

Requirements
  • Qualified Nurse with experience working as Band 6 within an acute specialism or Band 5 with long nursing experience and evidence working in an acute setting.
  • Experience of supervising junior staff and some managerial experience.
  • Ability to write reports and use IT, with experience of clinical supervision and teaching and presentation skills.
  • Knowledge of coagulation, history taking, and assessing, with experience of IV administration, venepuncture, and cannulation, CVA access.
