Job Title: SCPHN School Nurse Band 6

Transforming Health and Care with HCRG Care Group

We are seeking a highly skilled and compassionate SCPHN School Nurse to join our team in Central Lancashire. As a School Nurse, you will play a vital role in promoting health, wellbeing, and providing essential care to children and young people.

Main Responsibilities:
  • Deliver the Healthy Child Programme, assessing health needs, developing, implementing, and evaluating packages of care.
  • Use the Family Health Needs Assessment tool to plan and evidence care.
  • Develop and implement individualized care plans for each child, actively involving the child/parent/carer/family in the planning and implementation of care and support.
  • Participate in effective partnership working with other agencies and take a lead professional role where appropriate.
  • Use evidence-based practice to develop and maintain a high-quality and cost-effective standard for Specialist Public Health Nurse/School Nursing practice.
