We are seeking a highly skilled Clinical Pharmacist to support the Strawberry Hill Medical Centre team.

This role will involve providing specialist knowledge and expertise in medication monitoring, ensuring a safe and efficient repeat prescribing system, and collaborating with practice staff to develop a robust medication review policy.

Main Responsibilities:

  1. Ensure the practice repeat prescribing system is safe, efficient, and minimizes waste.
  2. Provide specialist knowledge on medication monitoring and ensure a system for implementation is in place.
  3. Answer queries from administration staff regarding medication requests, where appropriate, to save GP time.
  4. Reauthorize medication for future repeat prescribing within the scope of competence as an independent prescriber.
  5. Work with practice staff to ensure a robust medication review policy.
  6. Identify patients suitable for repeat dispensing and ensure its effective implementation.
  7. Review prescribing of medication for vulnerable patients to ensure it is appropriate.
  8. Ensure all safeguarding incidents are reported to the nominated Practice Safeguarding Lead.
  9. Review communication processes between practices, hospitals, care homes, and community pharmacies at hospital admission.
  10. Reconcile medicines following discharge from hospital or intermediate care, identify and rectify unexplained changes, and manage these changes without referral to the GP.
  11. Perform a clinical medication review, produce a post-discharge medicines care plan, and work with patients and community pharmacists to ensure patients receive the necessary medicines.
  12. Reconcile medication from clinic appointment correspondence, identifying any unexplained changes and rectifying these by communication with the secondary care provider.
  13. Provide patients with advice on treating self-limiting conditions through face-to-face or telephone consultations and patient information leaflets or website.
  14. Answer medical information queries from GPs, nurses, practice staff, patients, and third-party providers.
  15. Provide education on medicines-related topics for GPs, nurses, medical students, and registrars.
  16. Review NICE and other evidence-based guidance as it is issued, to evaluate any impact on medicines and prescribing.
  17. Provide regular education sessions for the clinicians at the practice to discuss audit results and agree an action plan for improving standards within clinical areas.
  18. Provide advice on suitable alternative medications in the event of a medicine being unavailable.
  19. Encourage cost-effective prescribing within the practice, including adherence to local formulary.
  20. Build good working relationships with local Medicines Management Team members.
  21. Provide education for the clinical team on new medicines.
  22. Review all relevant safety alerts and ensure any action required is undertaken.
  23. Work with the practice team to help reduce medicines-related emergency admissions.
  24. Carry out face-to-face clinical medication reviews for patients on multiple medications for long-term conditions.
  25. Review prescribing for patients under their care, ensuring it is appropriate, cost-effective, and safe, and identify unmet need, stop unnecessary or ineffective medicines.
  26. Contribute to achievement of QOF targets to improve health outcomes.
  27. Identify and visit housebound patients who would benefit from a clinical medication review or review of their LTC by a clinical pharmacist.
