Social Care Assistant

We are seeking a highly motivated and experienced Support Worker to join our team at the Mackenzie Resource Centre. As a Reablement Support Specialist, you will play a vital role in supporting older adults to remain at home in their community, focusing on reablement and community care.
Key Responsibilities:
- Work alongside social care and healthcare colleagues to deliver a programme of input for individuals attending the Mackenzie Centre.
- Support individuals with reablement needs, promoting independence and well-being.
- Develop and maintain effective relationships with service users, families, and colleagues.
- Contribute to the development and implementation of reablement plans, ensuring a person-centred approach.
- Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements:
- Experience of working in a social care or healthcare setting, with a focus on reablement and community care.
- Strong communication and interpersonal skills, with the ability to build effective relationships.
- Ability to work collaboratively as part of a multidisciplinary team.
- Knowledge of the reablement approach and person-centred care principles.
- Ability to maintain confidentiality and handle sensitive information.
