Job Overview:
We are seeking an experienced Social Prescribing Navigator to join our team at Beacon Medical Group. As a Social Prescribing Navigator, you will play a key role in connecting patients with local resources and services that address their non-medical needs. You will work closely with healthcare professionals to identify areas where patients require additional support and develop strategies to address these challenges.

About the Role:
This is an exciting opportunity to join a dynamic team dedicated to improving patient outcomes through social prescribing and health coaching. As a Social Prescribing Navigator, you will be responsible for:
- Identifying and engaging with patients who may benefit from social prescribing and health coaching
- Developing and implementing personalized care plans that incorporate social prescribing and health coaching principles
- Building relationships with local organizations and community groups to leverage resources and services for patients

Main Responsibilities:
- Conduct thorough assessments of patients' social determinants of health and develop targeted interventions to address identified needs
- Collaborate with multidisciplinary teams to ensure seamless patient care and effective communication
- Provide education and training to colleagues on social prescribing and health coaching best practices
- Develop and maintain knowledge of local resources and services, with ability to adapt to changing circumstances

Requirements:
- Relevant qualifications or experience in social work, health coaching, or a related field
- Excellent communication and interpersonal skills, with ability to build rapport with patients from diverse backgrounds
- Strong problem-solving and critical thinking abilities, with capacity to analyze complex situations and develop effective solutions
- Ability to work independently and as part of a collaborative team environment
- Familiarity with local resources and services, with willingness to learn and adapt to new information
- DBS check required
