We are seeking a highly motivated and experienced Cardiology Assistant Practitioner to join our team at Powys Teaching Health Board. As a key member of our Community Cardiology service, you will play a vital role in delivering high-quality care to patients with cardiac conditions.
Key Responsibilities- Support the provision of Cardiac Rehabilitation in North and Mid Powys, working closely with a multi-disciplinary team to provide evidence-based care to patients.
- Manage your own caseload, setting up Event Monitoring devices for patients and collecting and entering National Data for the Cardiac Rehabilitation Service (NACR).
- Undertake patient assessments, implement care programs, and report regularly to Registered Practitioners, recognizing your own limitations and seeking advice when necessary.
- Develop effective relationships with patients, their families, carers, and colleagues, and contribute to a supportive and progressive work environment.
- Level 4 vocational qualification in a relevant Health Care associated subject or Certificate in Health Care Support Worker Education.
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
- Experience of providing relevant clinical care, interpersonal and communication skills, and the ability to manage/diffuse challenging or threatening situations.
- Ability to work in a multi-agency arena, manage your own time and workload, prioritize work, delegate, and seek assistance when required.
- Excellent verbal and non-verbal communication skills, and the ability to actively listen and communicate sensitive or difficult information in a clear and compassionate manner.
- Local knowledge of statutory and voluntary services.
- Teaching or Assessment qualification.
- Experience in a range of clinical skills, including administration of medicines via oral, topical, SC, IM routes, and skills in non-complex wound care.
