About the Role

We are seeking an experienced Assistant Practitioner to join our team in delivering high-quality Community Cardiology services in North and Mid Powys. As an Assistant Practitioner, you will play a vital role in supporting the provision of Cardiac Rehabilitation in our community settings.

Key Responsibilities

  • Support the delivery of Cardiac Rehabilitation services in North and Mid Powys
  • Assist in setting up Event Monitoring devices for patients and collecting and entering National Data for the Cardiac Rehabilitation Service (NACR)
  • Complete onward referrals for patients who have completed Cardiac Rehabilitation
  • Manage your own caseload independently, carrying out tasks delegated by a Registered Practitioner with appropriate supervision
  • Provide person-centred care in accordance with current evidence and the organisation's policy and procedures

Requirements

  • Level 4 vocational qualification in a relevant Health Care associated subject or Certificate in Health Care Support Worker Education
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
  • Evidence of continuing personal development relevant to the role in health care
  • Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and the All Wales Band 4 Assistant Practitioner Governance Framework

Desirable Qualifications and Experience

  • Local knowledge of statutory and voluntary services
  • Teaching or Assessment qualification
  • Experience in a range of clinical skills
