Job Overview:

Thirteen Group is seeking a Support Service Coordinator to join our Care and Support Team. This role involves working with vulnerable client groups, including young families, to provide support and enable them to reach their potential.

Key Responsibilities:
  • Develop and implement person-centred support plans to meet the needs of service users.
  • Assess suitability for the service and provide written reports.
  • Manage risk and safeguarding for all residents, including complex families.
  • Work with individuals to maximise their income and apply for relevant welfare benefits and grants.
  • Collaborate with other teams to provide a comprehensive support service.
  • Manage void properties and allocations, ensuring efficient use of resources.
  • Responsible for building compliance and health & safety of the scheme.
Requirements:
  • Experience of supporting vulnerable client groups, including adult and child safeguarding.
  • Ability to thrive in a lone working environment and as part of a team.
  • Emotional resilience and ability to deal constructively with challenging behaviour.
  • Driving licence and access to a vehicle for outreach work.
