About the Role

We are seeking a highly skilled and enthusiastic Senior Physiotherapist to join our Integrated Network Team (INT) in Bristol, North Somerset, or South Gloucestershire. As a Senior Physiotherapist, you will play a key role in delivering exceptional community-based therapy, improving quality of life for service-users, and committed to rehabilitation in a home environment.

Key Responsibilities
  1. Conduct initial holistic assessments, implement, and evaluate programmes of care for patients with complex needs.
  2. Use clinical reasoning skills to assess, plan, implement, and evaluate patient-centred intervention programmes, promoting high-quality and harm-free care.
  3. Manage your caseload proactively and allocate work efficiently to ensure effective service coordination.
  4. Develop and maintain strong working relationships with service users, carers, and family members to identify achievable goals and maximise quality of life and independence.
  5. Collaborate with other services to facilitate timely discharges and prevent avoidable admissions.
  6. Assess and arrange the provision of equipment within scope of practice and report any issues.
  7. Take responsibility for the development of other team members, including peers, junior staff, and students.
  8. Maintain accurate and informative records and actively participate in training and professional development.
  9. Identify and mitigate risks to self, colleagues, and service users.
