We are seeking a dedicated and passionate Key Worker to join our team at Bath and North East Somerset Council. As a Key Worker, you will provide intensive support to young people aged 16-25 with Special Educational Needs and Disabilities (SEND), helping them access educational opportunities and prepare for adulthood.
Key Responsibilities:
- Provide individualized support to young people with SEND, focusing on their strengths, needs, and aspirations.
- Develop and implement personalized action plans to promote educational and personal development.
- Support access to volunteering, work experience, and educational opportunities that align with young people's interests and goals.
- Act as the primary point of contact for young people and their parents/carers, providing guidance and support as needed.
- Collaborate with a team of professionals to deliver holistic support, ensuring a seamless and effective service.
- Attend and contribute to multi-agency meetings to discuss young people's progress and develop strategies for improvement.
- Ensure high attendance rates and timely completion of assessments and action plans, maintaining accurate records and adhering to safeguarding and data protection protocols.
- Engage in one-to-one activities tailored to young people's needs, including vocational, PSHE, and enrichment activities that promote personal growth and development.
- Assist in exploring next steps beyond the programme, ensuring smooth transitions and supporting young people in their future endeavors.
