About the Role

We are seeking an experienced and skilled nurse practitioner to join our team at Northumbria Healthcare NHS Foundation Trust. As a Frailty Practitioner, you will play a vital role in providing high-quality care to our elderly patients, working closely with our multidisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Provide specialist clinical skills support to medical staff and the nursing team, working at the interface with Elderly Care consultants.
  • Develop and implement the Scope of Nursing Practice, providing nursing excellence in patient care for our Elderly population.
  • Triage patients, prioritizing clinical need, and initiating treatment plans in accordance with Trust guidelines and protocols.
  • Work autonomously assessing individual patient needs, initiating investigations, and determining a differential diagnosis.
  • Coordinate clinical and support services, maintaining or improving the service.
  • Role model compassionate and inclusive leadership, shaping the creation of a collective leadership culture within the trust.
Person Specification

We are looking for a candidate with:

  • 1st level registered nurse/AHP with substantial experience.
  • Registered Mentor.
  • Degree level study to Masters level or equivalent experience.
  • Advanced clinical skills qualification at level 7.
  • Non-medical prescribing qualification.
  • Demonstrate thorough understanding of expanded scope of professional practice issues.
  • Evidence of continuing academic and professional development - ENB998/730 or equivalent.
Desirable Qualifications and Experience
  • ALS/ILS.
  • IRMER course.
