Job Description

We are seeking a skilled and compassionate Social Services Coordinator to join our team. This role will involve providing support to patients, families, and carers in Lincolnshire.

Main Responsibilities:
  • Collection and analysis of patient data to inform service development needs.
  • Liaising with healthcare professionals to promote the service and manage referrals.
  • Working proactively to secure additional information for effective claim management.
Requirements:
  • Excellent communication skills.
  • Ability to work accurately and efficiently.
  • Strong organizational skills.

Salary: £28,000 - £32,000 per annum

This is a part-time position, working 16 hours per week on Thursdays and Fridays. The successful candidate will be required to travel as needed, so transport is essential.
