Deputy Home First Coordinator

About the Role
We are seeking an enthusiastic and motivated Deputy Home First Co-ordinator to join our multi-disciplinary Home First Team based in Wrexham. As a key member of the team, you will play a vital role in promoting the Home First ethos through ongoing therapeutic interventions in patients' own environments and on the Ready for Home wards.
Key Responsibilities
- Deputise for the Home First Co-ordinator and provide leadership and support to the team.
- Screen patients and identify care availability to facilitate safe and timely discharge from hospital.
- Work with the Community Resource Team to provide ongoing support to patients with complex needs.
- Act as a link between acute and community services, promoting effective communication and collaboration.
- Contribute to the development of the Home First service, identifying opportunities for improvement and implementing changes.
Requirements
- NMC Registered first-level nurse with significant community experience.
- Evidence of ongoing education and professional development to Diploma Level or equivalent level of experience.
- Excellent analytical, organisational, and communication skills.
- Ability to work collaboratively with members of the primary health care team and other agencies.
- IT skills and ability to use electronic patient records.
