About the Role: As a Support Practitioner, you will play a vital role in supporting vulnerable individuals, promoting their independence, and enhancing their quality of life. You will work closely with our multidisciplinary team to provide tailored support packages that meet the unique needs of each individual.

Key Responsibilities:
  • Provide social support to service users, including one-to-one support and group activities.
  • Assist service users with practical tasks, such as daily living skills and household chores.
  • Support service users to access community resources and services.
