Senior Patient Experience Coordinator

The Senior Patient Flow Coordinator will play a pivotal role in ensuring the smooth transition of patients from hospital to their preferred place of care, whether that be home, hospice, or care home. This involves actively supporting the discharge process and facilitating a robust discharge plan to reduce the length of hospital stay for patients receiving Fast-Track Continuing Health Care (CHC).
Key Responsibilities- Identify patients admitted to the hospital who are eligible for CHC and work closely with the healthcare team to develop a discharge plan.
- Collaborate with the Patient Flow Coordinators to maintain accurate and up-to-date data inputting, ensuring high-quality data is recorded.
- Supervise and support junior colleagues, providing guidance and training to ensure they are equipped to perform their roles effectively.
- Work closely with management to ensure trust policies and procedures are followed, and act as a role model for junior colleagues.
- Degree-level qualification in a relevant field or equivalent experience.
- GCSEs and good spoken and written English.
- Excellent communication and interpersonal skills.
- Effective decision-making and problem-solving ability.
- Experience using PC and computer systems.
