As a Community Care Provider, your primary responsibility will be to deliver crucial assistance to individuals needing help with daily activities, personal hygiene, and preserving their autonomy.

Provide personal care and daily living assistance as agreed upon with the service user, their family, and involved professionals. Adhere to the policies and procedures of Connected Health or as directed by Next of Kin. Foster and maintain professional relationships with service users and colleagues from diverse backgrounds. Communicate regularly with your immediate supervisor, especially regarding any changes in the service user's condition or circumstances. Utilize the on-call system as per service guidelines. Attend required training sessions, including induction, team meetings, and individual supervision. Complete necessary documentation, including service user records and timesheets. Comply with all Health and Safety Policies and Procedures. Participate in supervision, staff meetings, and training activities as needed. Take responsibility for enhancing your own knowledge and skills through experience and training. Accept additional responsibilities as assigned by the Service Manager following successful completion of specific training and personal development.
ESSENTIAL CRITERIA: No formal experience is required as full accredited training will be provided. You must possess an empathetic nature and be a driver with access to a vehicle.
