Clinical Pharmacist

We are seeking highly skilled Clinical Pharmacists to join our team at Medacs Healthcare. As a Clinical Pharmacist, you will play a vital role in providing the highest levels of patient care in our NHS hospital in Chelsea and Westminster.
Key Responsibilities:- Provide clinical pharmacy services to sub-specialties within the Emergency and Integrated Care Division and Planned Care (Surgery) Division.
- Review prescriptions for accuracy and legality, and identify actual and potential problems.
- Clinically screen and supply medicines suitable for use as an inpatient and appropriately labelled for discharge.
- Monitor and review the therapeutic use of medicines.
- Ensure the effective outcomes of treatment with medicines.
- Monitor patients for potential and actual adverse effects of their medicines.
- Support the planning of patient discharge with respect to medication.
- Ensure safe, clinically effective, and cost-effective use of medicines.
- Demonstrate professional accountability to patients.
- Support the provision of an efficient, safe, and legal dispensing service in the Dispensary as required.
- Master's degree in pharmacy (MPharm).
- Minimum 2 years' post-graduate experience.
