We are seeking an exceptional individual to join our Hospital Discharge Coordinator team, playing a crucial role in supporting the smooth transition of patients from hospital to community settings. As a highly skilled and compassionate Nurse, you will work collaboratively with colleagues to assess patient needs, develop comprehensive care plans, and ensure seamless handovers.

To succeed in this role, you will require a proven track record of working at B5 level in a relevant setting, with evidence of participation in audit and patient experience initiatives.
