Enablement Worker

Job Title: Enablement Worker - Reablement and Care at Home

Job Summary: We are seeking an experienced Enablement Worker to join our Nairn and Ardersier Reablement Team. The successful candidate will work closely with individuals and their families to assess and implement enablement plans, promoting independence and supporting timely discharges from hospital.

Key Responsibilities:

  • Assess and develop enablement plans with individuals and their families
  • Work collaboratively with healthcare professionals to ensure seamless care transitions
  • Provide education and support to individuals and their families to promote independence and self-management
  • Contribute to the development and implementation of care plans that meet the needs of individuals and their families

Requirements:

  • Registration with the Scottish Social Service Council
  • Experience working in a reablement or care at home setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
