Job Summary

We are seeking a highly skilled and dedicated Family and Networks Practitioner to join our team at Childwall and Wavertree Primary Care Network.

Key Responsibilities
  • Deliver high-quality direct patient care, making autonomous decisions in relation to presenting problems.
  • Screen patients for disease risk factors and early signs of illness, and develop care and treatment plans in consultation with patients.
  • Initiate investigations, review results, and make referrals to secondary care or other providers as appropriate.
  • Ensure appropriate follow-up of patients, record clear and contemporaneous computerised consultation notes, and participate in clinical audits and learning event analyses.
  • Prescribe generically whenever appropriate, and work with and provide support for other members of the primary health care team.
  • Play a lead role in the practice achieving agreed and other key performance indicator targets.
  • Undertake continual personal and professional development, adhering to organisational policies and procedures.
  • Establish strong working relationships with GPs and practice teams, and contribute to the wider aims and objectives of the PCN.
