Family and Networks Specialist

Job Summary:
We are seeking a highly skilled and dedicated Family and Networks Practitioner to join our team at Childwall and Wavertree Primary Care Network. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our organization.
Key Responsibilities:- Deliver Direct Patient Care: Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organization. Screen patients for disease risk factors and early signs of illness.
- Clinical Management: Cover all clinical management of patients in surgery as appropriate, including initiating investigations, reviewing results, making referrals to secondary care or to other providers as necessary. Develop care and treatment plans in consultation with patients and in line with current practice disease management protocols and wider guidance such as that from NICE.
- Follow-up and Record-Keeping: Ensure appropriate follow-up of patients and record clear and contemporaneous computerized consultation notes to agreed standards.
- Prescribing and Medication Management: Prescribe generically whenever appropriately and in accordance with the practice prescribing formulary, NICE guidance, and Good Prescribing Practice (as defined by British National Formulary guidance). Compile and issue computer-generated acute and repeat prescriptions, avoiding handwritten prescriptions.
- Professional Development: Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities, and provide evidence of learning activity as required.
- Teamwork and Collaboration: Work with and provide support for other members of the primary healthcare team, playing a lead role in the practice achieving agreed and other key performance indicator targets.
