Position Overview
The Care Plus Group is seeking a dedicated Healthcare Support Practitioner to join our dynamic team within the Community Inpatient Unit. This role is essential in assisting individuals on their journey to recovery and independence following health challenges.
Role Summary
As a Healthcare Support Practitioner, you will be part of a multi-disciplinary team, collaborating with Nurses, Physiotherapists, Occupational Therapists, Social Workers, and care staff to deliver high-quality support. Your primary responsibility will be to facilitate the smooth operation of patient care over a 24-hour period, ensuring that all physical, social, and emotional needs are met.
Key Responsibilities
- Coordinate and support patient care plans in collaboration with healthcare professionals.
- Assist in the admissions and discharges of service users, ensuring a seamless transition.
- Provide clinical and social care support, alongside practical duties related to the daily operations of the unit.
- Maintain accurate records and documentation in compliance with healthcare regulations.
- Engage in continuous professional development and training opportunities.
Qualifications
- A Foundation Degree or Higher Education Certificate (level 4) in Integrated Care or a related field, or a Level 3 qualification in health or social care with a commitment to progress to level 4.
- Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and Access.
- A minimum of two years of experience in a health or social care setting.
Skills and Attributes
- Strong communication skills with the ability to manage complex information.
- Ability to work effectively as part of a team while also demonstrating initiative.
- Understanding of patient needs and the importance of confidentiality.
- Knowledge of CQC regulations and the Mental Capacity Act.
