About the Job

We are currently seeking a skilled Community Case Manager to join our Adult Community Healthcare Team at Buckinghamshire Healthcare NHS Trust. This is an excellent opportunity for a dedicated professional to make a positive impact on the lives of patients and their families.

Job Description

The successful candidate will be responsible for delivering high-quality patient care and support, working closely with multidisciplinary teams to achieve optimal health outcomes. Key responsibilities will include:

  • Conducting comprehensive assessments of patients' physical and psychosocial needs
  • Developing and implementing individualized care plans, focusing on prevention, early intervention, and evidence-based practice
  • Coordinating care services with healthcare professionals, community organizations, and social services to ensure seamless transitions and effective care delivery
  • Providing education and support to patients, families, and caregivers on health management and self-care, empowering them to take control of their health
Requirements and Qualifications

To succeed in this role, you will need:

  • A registered nursing qualification (RGN or RNMH) or relevant experience as a case manager
  • Proven experience of working within a multidisciplinary team, with a focus on collaborative care delivery
  • Strong communication and interpersonal skills, with the ability to build relationships with diverse stakeholders
  • Ability to work independently and as part of a team, prioritizing tasks and managing competing demands
