Care Team Leader

Care UK is seeking a dedicated and compassionate Care Team Leader to join our team in Hythe, Southampton. This is an exciting opportunity for individuals who are passionate about providing exceptional care and support to residents.
In this role, you will be responsible for leading and supervising your shift, case managing residents, and liaising with families, healthcare professionals, and other stakeholders to ensure the best care plans are created and delivered. You will also administer and record medication, carry out risk assessments, and work closely with the leadership team to develop services.
We offer a competitive salary of £21,400 - £26,500 per annum, depending on experience. As a Care Team Leader, you will have access to excellent training and development opportunities, including a National VQ Level 3 qualification in Health and Social Care.
Key Responsibilities:
- Lead and supervise shifts to ensure high-quality care delivery.
- Case manage residents to identify and address their needs.
- Liaise with families, healthcare professionals, and other stakeholders to ensure seamless care coordination.
- Administer and record medication according to established protocols.
- Conduct regular risk assessments to prevent harm and promote resident safety.
About You:
- National VQ Level 3 in Health and Social Care or equivalent qualification.
- Proven ability to lead and supervise others.
- Excellent communication and interpersonal skills.
- Able to work effectively in a fast-paced environment.
