About the Role

We are seeking a highly motivated and experienced Adult Mental Health Practitioner to join our Ellesmere Port and Neston Community Mental Health Team. As a key member of our team, you will work closely with patients, families, and other healthcare professionals to provide high-quality care and support.

Main Responsibilities
  • Undertake assessments and coordinate care for patients with complex mental health needs
  • Develop and implement individualized care plans, including risk assessments and interventions to promote recovery and well-being
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Provide education and support to patients and their families to promote mental health and well-being
  • Participate in quality improvement initiatives to enhance patient care and outcomes
